Job Summary

 

A qualified Licensed Practical/Vocational Nurse administers skilled nursing care to patients in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Agency policies and procedures.

Responsibilities:

  • Assist in the development, implementation, evaluation and revision of plans of care

  • Implement the nursing plan of care for each assigned patient

  • Submit accurate patient documentation per Agency policy

  • Provide nursing services, treatments and diagnostic and preventive procedures as assigned

  • Observe signs and symptoms and reports to the physician and RN reactions to treatments and medications, and changes in the patient’s physical or emotional condition

  • Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home

  • Initiate preventive and rehabilitative nursing procedures as appropriate

  • Maintain accurate and complete records of observations, treatments and care of patient

  • Complete in-services as assigned. Attend staff meetings/conferences as scheduled

  • Perform treatments as ordered by the physician and in accordance with the plan of care

  • Observe aseptic technique as required

  • Provide extended hours of patient care

  • Ensure safe, quality, cost-efficient skilled nursing services

  • Perform other job duties as assigned

Requirements

  • Graduate of an accredited school of practical/vocational nursing and currently licensed in the state of Tennessee

  • Minimum of one (1) year of nursing experience required

  • Acceptance of philosophy and goals of this Agency

  • Ability to cope with the emotional stress of patients and their families

  • Willingness to work in home environments of patients who may have values or standards different from your own

  • Ability to perform venipuncture

  • Ability to turn, lift, carry and otherwise transport patient

  • Ability to communicate effectively, both verbally and in writing

  • Current CPR certification

  • Current Tennessee driver’s license and automobile liability insurance
